01

Define the helper's role with the person involved

Discuss what assistance is wanted, who is authorized to provide it and what the person receiving support wants shared. Do not turn an informal note into permission for procedures, decisions or access that have not been agreed. Identify the limits of the helper's training and role. Where clinical care or legal authority is involved, use the appropriate professional and formal documentation. This worksheet organizes information and handoff questions; it does not establish a treatment plan, capacity determination, guardianship or substitute decision-maker.

02

Describe the practical routine in plain language

Record the ordinary arrangements the authorized helper needs to understand: preferred communication, relevant accessibility needs, usual locations of essential equipment and the agreed way to obtain more information. Use the person's own preferences and current instructions rather than assumptions about age or diagnosis. Keep descriptions concise enough to follow. A long history may obscure the action the helper actually needs to take. Distinguish a preference from an instruction supplied by a qualified provider and identify the source of the latter. Do not simplify a professional instruction in a way that changes its meaning.

03

Keep professional instructions as identifiable sources

Record where the current care plan, equipment instructions and relevant provider contacts are held. Preserve the original wording and version where it matters, and resolve conflicting copies through the appropriate provider. Do not invent medication substitutions, doses, equipment settings or treatment changes to fill a blank on the sheet. If safe continuity cannot be provided by the planned helper, arrange the appropriate service or alternative location. A well-written handoff note cannot make an untrained person competent to perform a task outside their role.

04

Plan access without exposing the whole record

Decide how the authorized helper will obtain the necessary information and equipment if the usual caregiver is unavailable. Share only the information needed for that role and protect sensitive records appropriately. Keep passwords and full account details out of a sheet that may be carried or shown to several people. Where a protected digital record is used, test the authorized recovery method in advance without uploading private care information into a public support form or AI conversation. A record that exists but cannot be reached by the right person is an unresolved planning gap.

05

Use a clear handoff and return log

When a handoff occurs, record the date, people involved, information supplied and any unresolved question requiring professional clarification. Keep the person's participation and consent central where applicable. Do not treat a checkbox as evidence that every care task was completed correctly. On return, record relevant observations through the appropriate care process and update any practical planning gaps. If there is an urgent health or safety concern, seek the appropriate assistance rather than delaying to complete the paperwork. The log supports communication; it is not a clinical assessment.

06

Review after changes to people or instructions

Revisit the sheet when care needs, providers, equipment, helpers or contact details change. Confirm that the helper still agrees to the role and can actually provide the planned assistance. A calm review can involve explaining the routine and locating the current instructions without performing unfamiliar procedures. Remove conflicting outdated copies through an appropriate recordkeeping process. Keep a dated list of unresolved arrangements so the existence of a handoff sheet is not mistaken for complete care continuity. Coordinate the sheet with the household's transport, destination and professional backup plans.
